Moderate Deficiency (3.5-5 and asymptomatic)

Slow K or K Dur - 2 tabs PO BID

K-Lyte 25 mEq K PO BID

Severe Deficiency (less than 0.5 mmol or symptomatic)

10 mEq KCL in 100cc NS over 1h x 3 with maximum rate of infusion 10 mEq per hour

Moderate Deficiency (asymptomatic)

15-30ml Mg Rougier PO TID-QID

1-2 tab Mg Gluconate TID-QID

Severe Deficiency (less than 0.5 mmol or symptomatic)

2g IV Mg Sulf in 100ml NaCl or D5W at 1-2 g/h with rate not to exceed 150 mg/min (reduce dose in renal failure)

Moderate Deficiency (0.3-0.7)

15 mmol phosphate in 100-250 NaCl over 4 hours via central line

1-2 tabs Phosphate Novartis Po BID-TID

Severe Deficiency (less than 0.5 mmol or symptomatic)

30 mmol phosphate in 100-250 NaCl over 4 hours via central line with repeat serum phosphate level after administration

Moderate Deficiency

1-2 tabs calcium carbonate PO BID-TID

Severe Deficiency

1-2g calcium gluconate in 100ml NS at 1g/h rate not to exceed 0.75 mmol/min

1g calcium chloride in 100ml NS at 1g/h rate not to exceed 0.75 mmol/min
