Risk of Malignancy Index (RMI) Calculator
As part of clinically working up possible malignancies, the RMI score is often used to determine the likelihood that a mass/cyst seen on ultrasound warrants special consideration and/or consultation to gynecology oncology. This tool aims to simply the process. A score of 200 or more indicates a high malignancy risk.
This calculator is based off of the article Initial Evaluation and Referral Guidelines for Management of Pelvic/Ovarian Masses available through the SOGC.

Enter the most recent Ca 125 level (U/ml) for the patient.

In the RMI II scoring system, menopause is worth 4 points while ultrasound findings are worth 1 of one or zero findings or 4 for two or more findings. RMI II scoring system is recommended to provide an objective assessment of the underlying malignant potential, using a cut-off score of 200.

In the RMI I scoring system, menopause is worth only 3 points while ultrasound findings are worth 0 for zero findings, 1 for one finding, or 3 more two or more findings.
